Registered Nurse - Case Manager

Seeking an experienced Registered Nurse to support care coordination, utilization review, and discharge planning activities within the Emergency Department. The Care Manager will collaborate with interdisciplinary teams to ensure appropriate patient care, efficient resource utilization, and safe transitions of care. Key Responsibilities Coordinate patient care and discharge planning activities. Perform utilization review and assess appropriateness of services. Collaborate with physicians, nursing staff, social workers, and community resources. Facilitate safe patient transitions and continuity of care. Maintain accurate documentation and comply with regulatory requirements. Support patient-centered care and quality improvement initiatives. Requirements Experience with Care Coordination, Utilization Review, and Discharge Planning. Associate Degree in Nursing (ASN/ADN) from an accredited nursing program. Active New York State RN License with NYSED Primary Source Verification. Current BLS Certification. PRI and Screen Certification required within 3 months of employment.
