Key Responsibilities\n

• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations to establish treatment goals.

• Deliver targeted, brief interventions for patients requiring mental health services, adhering to all Agency and Federally Qualified Health Center (FQHC) guidelines.

• Assist primary care providers in identifying and managing mental health and psychosocial issues within the patient population.
\n
• Regularly follow up with patients to assess their progress and adjust treatment plans as necessary.
\n
• Document patient interactions, treatment plans, and progress in a clear and precise manner to maintain accurate clinical records.
\n
• Advise at-risk patients about preventive strategies and assist them in achieving stable functioning.
\n
• Facilitate connections with other healthcare providers, both within our facility and externally, with patient consent.
\n
• Work to effect behavioral changes in patients, particularly those with or at risk for physical disorders, to support healthier lifestyle choices.
\n
• Provide self-management skills and educational resources to empower patients to participate actively in their treatment journey.
\n
• Consult with clinic management and multidisciplinary team members regarding the behavioral services provided.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling.

• Current and valid LPC (Licensed Professional Counselor) license in Pennsylvania.

• Strong understanding of mental health assessment, treatment planning, developmental psychopathology, and psychotherapeutic techniques.

• Excellent communication skills and ability to establish effective relationships with patients, families, and other healthcare professionals.

Preferred Experience\n

Compensation and Benefits\n

The salary range for this full-time position is competitive, ranging from USD 87,000 to 105,000 annually, based on experience and qualifications.
