Key Responsibilities\n

• Act as a primary liaison between patients, families, and outpatient providers, ensuring seamless communication and a supportive transition through the care process.

• Participate actively in discharge and aftercare planning, ensuring continuity of care upon the patient’s transition from our hospice services.

• Provide therapeutic meal support, guiding clients toward a healthy relationship with food while supporting their emotional well-being.

• Deliver empathetic and tailored care to each client, respecting their unique set of circumstances and intersecting identities.

Patient Population / Client Focus\n

Our primary focus is on older adults facing terminal illnesses. As a Licensed Clinical Social Worker, you will be instrumental in addressing their mental health needs, providing a consistent and affirming presence during challenging times. Compensation and Benefits\n
