Key Responsibilities\n

As a Senior LCSW at our center, you will take on a range of responsibilities, including but not limited to:

• Conducting thorough biopsychosocial assessments that are age-appropriate and culturally sensitive, aimed at understanding the mental health, substance use, and medical needs of patients.

• Providing evidenced-based behavioral health services, such as individual and group counseling, intake assessments, and crisis intervention tailored to the complexities of hospice care.

• Documenting patient interaction and progress notes electronically, ensuring compliance with organizational standards and regulations.

• Collaborating with local community resources, healthcare providers, and support groups to enhance patient care and provide referral services, particularly for medication assistance programs.

Qualifications\n
