Bronx x6:

Monday - Friday 9AM-5PM x3

Tuesday - Saturday 9AM-5PM (scatter site) x3

**We do not have any part time or remote/hybrid schedules available at this time**

Summary : The Wellness Coach works independently in the field, and as part of a rehabilitation services delivery team, with people of diverse ethnicities, backgrounds and preferences who have a mental illness and are living in the community, and scatter-site apartment.

Responsibilities :

• Engage people with mental illness and assist them in meeting the demands of community living and maintaining their housing.
• Assist in teaching individuals the necessary skills, and linking them to services that will support their recovery, independence, and self-determination.
• Conduct Group Meetings on Wellness Management.
• Complete service plan reviews for all residents.
• Regularly assess, correct and report apartment and personal living space inspections conditions as assigned.
• Provide Case Management for residents, engage residents in setting rehabilitation and housing support goals, and collaborate with them in developing a Housing Support Plan where indicated.

Requirements :

• 2-3 years of experience working within the Human Services field
• Bachelor's Degree - required
• Bilingual Spanish - preferred

A little about the role:
These are part-time/full-time temporary to permanent role lasting 5 to 8 months, based on the client’s needs. After the contract ends, the client will decide whether to offer a permanent position, at which point pay and benefits would transfer to them. During the contract period, your pay and benefits will be provided through Soloh Partners. You'll be paid weekly on Fridays, and benefits include health, vision, dental, 401(k), and paid sick time off. Company Description
