Adult Mental Health Case Manager

Role Description The Adult Mental Health Case Manager is a full-time, on-site role located in Ocean, NJ. This role is responsible for coordinating and overseeing services for adults receiving care in the day treatment program, including developing individualized service plans, monitoring progress, and adjusting supports as needed. Day-to-day tasks include conducting intake assessments, facilitating referrals to community resources, coordinating with clinicians and external providers, and advocating for client needs. The Case Manager will maintain accurate documentation, participate in interdisciplinary team meetings, and ensure compliance with program standards and regulatory requirements. The role also involves providing ongoing support, psychoeducation, and crisis intervention within the scope of practice to help clients achieve treatment goals and maintain stability in the community. Qualifications Demonstrated experience in adult mental health, case management, or social services, preferably in a day treatment or community-based setting. Strong skills in assessment, care planning, service coordination, and advocacy for adults with mental health needs. Ability to maintain thorough, timely documentation and manage caseloads in accordance with regulatory and agency standards. Excellent interpersonal, communication, and collaboration skills for working with clients, families, and multidisciplinary teams. Knowledge of local community resources, behavioral health systems, and principles of person-centered and trauma-informed care. Bachelor’s degree in social work, psychology, counseling, human services, or a related field; relevant licensure or certification is a plus.
