Key Responsibilities\n

• Psychosocial Assessments: Conduct thorough and timely evaluations of patients’ psychosocial situations, utilizing your understanding of human behavior to identify factors that may impact treatment outcomes.

• Clinical Interventions: Provide supportive counseling to patients and families, linking them with appropriate community resources, facilitating family meetings, and assisting with complex decisions related to treatment options, such as hospice or DNR protocols.

• Patient Care Management: Manage a dynamic clinical caseload by identifying high-risk situations and coordinating ongoing care, collaborating closely with case managers to prepare for effective patient discharges.

• Legal and Ethical Compliance: Adhere to all legal and ethical standards during assessments and interventions, incorporating policies related to patient rights and safety, such as duty to warn and reporting suspected abuse.

• Documentation: Maintain accurate and comprehensive records as per organizational policies, including progress notes and interdisciplinary care plans.

• Staff Training: Conduct in-service training sessions for medical staff to improve understanding of psychosocial issues and enhance patient care strategies.

• Quality Improvement: Participate in initiatives aimed at improving patient care through the development and analysis of relevant indicators.

• Additional Duties: Engage in other assigned activities, which may include planning events for Social Work Month or developing support groups for cancer patients and their families.

Qualifications\n

Compensation and Benefits\n

The annual salary for this position ranges from $120,000 to $130,000, commensurate with experience and qualifications.
