Housing Stability Lead — Case Management & Advocacy

The Housing Stability Coordinator (HSC) at Housing Visions supports individuals enrolled in NYS 1115 Medicaid Waiver programs. You assess health-related social needs, including housing stability, food security, utilities, and transportation, and provide person-centered case management and advocacy to improve health and housing outcomes.
