Key Responsibilities

• Develop and implement individualized treatment plans based on thorough evaluations and assessments of each patient's physical capabilities and rehabilitation goals.
• Conduct regular assessments of patient progress, adjusting treatment plans as necessary to optimize recovery outcomes.
• Utilize various therapeutic modalities, exercises, and hands-on techniques to facilitate patient healing, pain relief, and functional improvement.
• Oversee and mentor Physical Therapy Assistants, ensuring they provide care that aligns with best practices and the established plan of care.
• Engage with patients in their rehabilitation journey, providing education, motivation, and support to encourage active participation in their therapy.
• Evaluate the patient's home environment when necessary to recommend modifications that can improve their independence and safety during recovery.
• Maintain accurate and timely clinical records, documenting evaluations, treatment plans, progress notes, and discharge summaries to ensure comprehensive patient care and compliance with regulatory requirements.
• Participate in team meetings and case conferences, contributing insights and adjustments to treatment plans based on collaboration with interdisciplinary team members.

Qualifications
