We are seeking a dedicated Full-Time Physical Therapist to join Swift Health Physical Therapy, located in the scenic Front Royal, Virginia. Work Environment

This position is performed onsite, primarily within the homes of our patients in the Front Royal area. Key Responsibilities

• Conduct thorough evaluations, which include reviewing patient histories and performing comprehensive assessments to identify treatment needs.
• Develop and explain individualized treatment plans based on evaluation findings, incorporating feedback and goals from both patients and their families.
• Implement treatment protocols that respond to each patient's specific conditions and progress, ensuring high standards of care are maintained.
• Consult with a multidisciplinary team of healthcare professionals to ensure coordinated and holistic care, adapting the plan as necessary based on patient responses.
• Document all patient interactions and treatment efficacy accurately, providing clear progress notes that facilitate ongoing patient care.
• Manage your time effectively to meet productivity expectations while maintaining an organized schedule and being punctual for appointments.
• Participate in community outreach and education initiatives, promoting physical therapy services and supporting broader health initiatives.
• Take an active role in your professional development by attending continuing education programs and serving as a clinical instructor for students.
• Strive to enhance patient experiences and accessibility through adaptable and patient-centered approaches.

Patient Population / Client Focus

Our Physical Therapists engage with a variety of patient populations, ranging from post-operative recovery individuals to those managing chronic pain and mobility issues. By providing care within the home setting, therapists gain insights into the unique challenges faced by each patient, enabling tailored treatment plans that promote recovery in a familiar environment.

Qualifications

• Bachelor's degree in Physical Therapy.
• A valid Physical Therapist license in Virginia to practice, as well as adherence to any state-specific licensing requirements.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• Minimum of 2 years of relevant experience in a clinical setting; however, newly graduated Therapists with relevant clinical training are encouraged to apply.

Preferred Experience

• Experience in home health settings with knowledge of managing diverse patient needs is advantageous.
• Familiarity with electronic health record (EHR) systems and documentation practices.
• Previous involvement in patient education and community health initiatives.

Compensation and Benefits
