Work Environment

This role is primarily onsite, focusing on home care settings. You'll work directly in the community, providing rehabilitation services in patients' homes. This flexible model enables us to provide tailored care that aligns with each patient's specific needs and rehabilitative goals. Our team supports a healthy work-life balance, offering a schedule that accommodates personal commitments while fulfilling the requirement for 40 hours of work per week.

Key Responsibilities

• Deliver high-quality, evidence-based orthopedic care in patients' homes, ensuring their safety, comfort, and rehabilitation.

• Develop and implement personalized treatment programs based on comprehensive assessments and the established treatment plans designed by senior physical therapists.

• Conduct thorough evaluations, including reviewing medical histories, performing specific physical assessments, and documenting findings to create actionable care plans.

• Manage discharge planning and aftercare, ensuring patients receive proper guidance and support post-treatment, while meticulously documenting all patient interaction and care activities.

• Maintain compliance with all relevant clinical regulations and organizational policies, ensuring you complete all required corporate training promptly.

• Communicate effectively with interdisciplinary teams, including physicians, nursing staff, and family members, promoting a holistic approach to patient care.

• Engage in regular meetings and training sessions directed by management, enhancing team cohesion and clinical knowledge.

Patient Population / Client Focus

In this role, you will primarily serve orthopedic populations, including patients recovering from surgeries such as joint replacements, fractures, and sports-related injuries. Your expertise will guide patients through their rehabilitation journeys, helping restore mobility and function while minimizing pain and facilitating a speedy recovery. Our practice emphasizes individualized care, which allows our therapists to create meaningful relationships with each patient, ultimately contributing to better health outcomes.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.

• Active and unrestricted physical therapy license as required in Virginia.

• Current American Heart Association BLS for Healthcare Providers CPR certification.

• Strong interpersonal skills, with the ability to build rapport and trust with patients and collaborate effectively within a healthcare team.

Preferred Experience

A minimum of two years of experience in a physical therapy setting, ideally with a focus on orthopedic rehabilitation. Experience in home health or community care settings is highly desirable.

Compensation and Benefits
