Key Responsibilities\n

In this role, you will report to the Social Work Manager and have the following responsibilities:

• Evaluate and diagnose a variety of psychosocial issues that may hinder patients from achieving optimal mental health.

• Conduct comprehensive bio-psychosocial assessments to identify barriers to effective treatment and enhance patient engagement.

• Develop individualized treatment plans utilizing evidence-based practices, tailoring interventions to meet the unique needs of each patient.

• Participate in multidisciplinary team meetings, providing insights into patients' social backgrounds, psychosocial functioning, and needs to enhance care coordination.

• Document all social work activities and patient progress in medical records, ensuring compliance with departmental standards.

• Provide educational resources and counseling to patients and families, assisting them in understanding medical and social work recommendations.

• Advocate for patients’ needs in the community and facilitate access to social services, emphasizing empowerment and autonomy.

• Maintain up-to-date knowledge of community resources to effectively connect patients with necessary support.

The patient population you will serve includes individuals of various ages and backgrounds seeking mental health support. Qualifications\n

The ideal candidate must possess the following qualifications:

• A Master’s Degree in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in Texas.
