Responsibilities:

• Develop a comprehensive social history, social assessment, and care plan for each new resident, identifying areas of need and implementing effective strategies for resolution

• Collaborate with residents, families, and interdisciplinary teams to ensure individualized care and support

• Provide counseling and support to residents and families, promoting positive relationships and effective communication

• Participate in care plan meetings, evaluating the effectiveness of interventions and revising plans as needed

• Assist with resident discharge and placement planning, ensuring a smooth transition to the next stage of care

Requirements:

• Currently licensed social worker per State of Nebraska requirements

• Minimum of two (2) years social work experience in a healthcare-related field

• Ability to maintain regular attendance and complete Social Services Certification through The State of Nebraska

• Must possess computer skills, good organizational and time management skills, and the ability to maintain confidentiality

• Excellent communication and interpersonal skills, with the ability to work cooperatively with residents, families, and staff
