A well established long term care organization is seeking an experienced Regional Social Worker to provide leadership, guidance, and support to facility-based social service teams across multiple skilled nursing centers. The ideal candidate is a compassionate professional with a strong understanding of regulatory requirements, psychosocial care, and interdisciplinary collaboration in the long-term care environment.

Responsibilities of Regional Social Worker

• Provide regional oversight, mentorship, and training to facility social workers and department leaders.

• Ensure compliance with federal, state, and local regulations governing social services in skilled nursing facilities.

• Support facilities in resident assessments, care planning, and discharge processes.

• Collaborate with clinical, admissions, and administrative teams to promote quality outcomes and smooth transitions of care.

• Conduct on-site visits to monitor department performance and assist with survey readiness.

• Serve as a resource for complex psychosocial cases and family concerns.

• Contribute to the development of company-wide policies and best practices for social services.

Qualifications of Regional Social Worker

• Bachelor’s degree in Social Work (BSW) required; Master’s degree (MSW) preferred.

• Current Social Work licensure as required by PA and NJ

• Minimum of 5 years of experience in long-term care, ideally at least 2 years in a leadership or multi-site role.

• Strong knowledge of discharge planning, resident rights, care coordination, and documentation standards.

• Excellent communication, organization, and leadership skills.

• Ability to travel regionally between assigned facilities.
