Key Responsibilities

• Conduct comprehensive assessments to understand the social and emotional needs of patients and their families, establishing individualized treatment plans that demonstrate medical necessity and align with our program's criteria.
• Utilize telehealth technologies to deliver mental health services, ensuring access to care for individuals dealing with trauma and substance use disorders.
• Research, identify, and connect clients with community resources such as food assistance, childcare services, and healthcare options, advocating on their behalf to meet their unique needs.
• Respond promptly to crisis situations, including child abuse cases and mental health emergencies, providing immediate support and intervention as necessary.
• Regularly monitor clients' progress through follow-up assessments and consultations, adjusting treatment plans to improve outcomes effectively.
• Assist clients in navigating life changes and challenges such as health crises, relationship transitions, or economic hardships.

Patient Population / Client Focus

This role primarily focuses on supporting individuals in hospice care, including patients with terminal illnesses and their families. A strong emphasis is placed on enhancing the emotional well-being and quality of life for those facing significant life transitions. Qualifications

• A Master’s Degree in Social Work (MSW) from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in Pennsylvania.
• At least 2 years of experience providing mental health services, preferably in a health-related setting.
• Experience working with Veterans and understanding their specific needs is highly desirable.
• Comprehensive knowledge of mental health conditions and best practices for psychological treatment.

Preferred Experience

Compensation and Benefits
