Job Summary This position is responsible for delivering high-quality nursing care and managing the overall coordination of home-based services for assigned patients. Using an interdisciplinary team approach, the RN will assess, plan, implement, and evaluate individualized care, ensuring continuity, quality outcomes, and optimal patient and family support. This position may begin as aVisiting RN, transitioning into aCase Managerrole once a half caseload is assumed, in accordance with the Collective Bargaining Agreement with Montana Nurses Association Local #32. Both roles share the same clinical responsibilities. Essential Duties and Responsibilities: Provides skilled nursing and coordinates care to patients and family members in a residential setting through a team approach and based upon systematic evaluation as authorized by a physician. Observes, assesses, evaluates, and plans interventions in relation to patients physical and psychosocial statuses, including changes in condition and/or reactions to treatments or medications for the duration of contact with patients. Oversees care provided by Home Health Aides and CNAs to assigned patients and evaluates compliance with plan of care. Assures continuity of care through effective communication, information exchange and case conferences with other disciplines, medical providers, community health practitioners, support and social agencies. Provides quality care and appropriate symptom management for positive clinical outcomes and promotes optimal health and quality of life through teaching, counseling, appropriate interventions and referrals. Maintains accurate, current and complete patient records in accordance with Agency standards and OASIS/HIS/HOPE requirements as well as knowledge of current reimbursement rules and regulations. Participates in team scheduling coordination and on-call rotation schedule as necessary to meet patient care needs by assessing and evaluating patient needs via telephone and advising or making home visits as required. Participates in quality improvement activities, acquires knowledge and accepts responsibility of quality metrics related to care delivery and attends in-service training as assigned. Manages and meets productivity and caseload expectations in line with the agencys standards. Performs other duties as assigned by supervisor. Schedule Typical workdays areMonday through Friday, with evening/weekendon-call rotation. Schedule may vary depending on FTE status and patient care needs, as directed by the supervisor.
