Key Responsibilities\n

In your role as a Licensed Clinical Social Worker, you will take on a variety of responsibilities that directly impact patient care and organizational success. Your primary duties will include:

• Conducting comprehensive biopsychosocial assessments tailored to meet the mental health, substance abuse, and medical needs of clients in a primary care context.

• Providing individualized and group counseling services to a diverse client base, including intake assessments, treatment planning, crisis intervention, and referrals to specialized care as needed.

• Documenting care and follow-up assessments electronically, ensuring that progress notes and suicide risk assessments are accurate, timely, and meet regulatory requirements.

• Enhancing client support through liaison services, connecting them with local resources, community programs, and medication assistance initiatives to bolster their treatment journey.

• Collaborating with Quality Improvement teams to devise and implement effective processes for data collection, ultimately enhancing the quality of care provided to our patients.

Qualifications\n

To thrive in this role, candidates must meet the following qualifications:

• Hold a current and valid state license as a Licensed Clinical Social Worker (LCSW).
