The full-time work schedule typically runs Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours as needed to accommodate clients. All social workers must be flexible and available, potentially handling urgent calls and crises outside of regular hours.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, evaluating mental, emotional, and social factors that impact their health and well-being.

• Collaborate closely with clients and their families to develop individualized care plans that address each client's unique challenges, preferences, and aspirations.

• Provide counseling and emotional support to seniors, assisting them through significant life transitions such as bereavement, health changes, and relocation.

• Advocate for clients' rights and connect them with community resources, ensuring they receive adequate support, including housing, medical care, and social services.

• Engage with interdisciplinary teams, including nurses and healthcare professionals, to create a holistic approach to client care, ensuring seamless service delivery.

• Perform regular follow-ups and home visits (virtually) to monitor clients' status, assess safety, and identify changing needs or challenges.

• Facilitate effective communication among family members, helping to resolve conflicts and promoting a supportive care environment.

• Educate clients on available community resources, support groups, and programs that foster social engagement and enhance their quality of life.

• Maintain thorough documentation of assessments, care plans, and ongoing progress, adhering to compliance and ethical standards.

This position focuses primarily on the senior population within Halethorpe and surrounding areas, addressing their unique mental health needs. Qualifications\n

• A Master’s degree in Social Work from an accredited academic institution.

The position offers a competitive annual salary ranging from USD 60,000 to 100,000, commensurate with experience.
