Key Responsibilities\n

• Provide intensive case management services tailored to a small caseload of children and their families, ensuring effective and personalized care.

• Facilitate scheduling and attendance for patients at medical, behavioral health, and specialist appointments to enhance their continuity of care.

• Provide comprehensive administrative support, including records retrieval, eligibility screenings, and goal tracking to optimize patient outcomes.

• Coordinate all necessary care with patients to ensure a seamless experience through our healthcare system.

• Conduct home and community visits as warranted, adapting strategies to meet patients where they are.

• Collaborate with an interdisciplinary care team, including behavioral health experts, medical providers, and external partners, to foster integrated care solutions.

• Document all patient interactions meticulously using our electronic health record and supplementary databases to enhance accountability and service quality.

Qualifications\n

Compensation and Benefits\n

The compensation for this position ranges from $80,000 to $90,000 annually, commensurate with experience and qualifications.
