Work Environment

This position operates primarily in a home care setting, which allows therapists to create personalized rehabilitation plans in the comfort of patients' homes. The role requires strong interpersonal skills, as building rapport with patients and their families is essential. The typical working schedule is Monday to Friday, from 9:00 am to 5:00 pm, offering a consistent work-life balance conducive to personal well-being.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities and limitations to develop tailored treatment plans that address their unique needs.
• Administer therapeutic exercises, manual therapy techniques, and modalities such as ultrasound or electrical stimulation to promote recovery and relieve pain.
• Follow evidence-based practices to ensure patients receive the most effective care, continually assessing their progress and modifying treatment plans as necessary.
• Educate patients and their families about therapeutic exercises, proper body mechanics, and preventive measures to minimize the risk of re-injury.
• Maintain meticulous documentation of patient assessments, treatment interventions, and progress notes in compliance with regulatory standards.
• Collaborate with a team of healthcare professionals, including nurses and occupational therapists, to create comprehensive care strategies that enhance patient outcomes.

Patient Population / Client Focus

The ideal candidate will work with a variety of patient demographics, including those recovering from surgery, injury, or chronic conditions affecting mobility. Our patients typically vary in age and abilities, from post-operative orthopedic patients to those requiring assistance for neurologic impairments. Understanding the nuances of each patient's needs is crucial to providing effective care that yields positive results.

Qualifications

• A valid New York State Physical Therapist License is required.
• A Bachelor's degree in Physical Therapy (MSPT or DPT) from an accredited program is essential.
• A minimum of two years of experience in a clinical setting, particularly in rehabilitation or home care environments, is highly preferred.

Preferred Experience

Experience in a skilled nursing facility or home care setting is preferred, as well as familiarity with electronic health records (EHR) systems for efficient documentation. Additional certifications in specialized techniques, such as manual therapy or sports rehabilitation, will be considered an asset.

Compensation and Benefits

This position offers an annual salary ranging from $100,000 to $120,000, commensurate with experience and education.
