This role is primarily remote, allowing for flexibility while maintaining accessibility to our patients. As a part of our virtual clinical team, you will collaborate with health care professionals through digital platforms, ensuring seamless communication and continuous support. Key Responsibilities\n

• Design treatment plans in collaboration with our multidisciplinary health care team, focusing on psychosocial assessments and employing accepted social work interventions.

• Establish and maintain professional relationships with community leaders and organizations, representing our clinic in meetings and committees tackling local social needs affecting our patient population.

• Conduct crisis interventions and therapeutic counseling, offering assessments and tailored treatment for emotional and behavioral challenges faced by clients.
\n
• Maintain proactive communication with physicians and other healthcare providers regarding patient progress, emergent needs, and potential concerns, contributing to integrated treatment plans.

• Ensure meticulous upkeep of clinical records and progress notes in-line with departmental policies, facilitating accountability and transparency in patient care.

Patient Population / Client Focus\n

Your primary role will be to provide mental health support to a diverse range of clients in our outpatient setting, addressing various psychological and emotional issues. Clients will include children, adolescents, adults, and families dealing with stress, anxiety, depression, trauma, and other mental health challenges. Your work will directly impact their quality of life and overall well-being.

Qualifications\n

• A Master’s degree in Social Work or equivalent from an accredited institution, with current licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of three years of experience in an outpatient mental health setting, demonstrating a solid foundation of clinical skills and community outreach.

• Exceptional interpersonal skills, with the ability to cultivate professional relationships with colleagues, medical staff, and clients alike.

Preferred Experience\n

Compensation and Benefits\n
