Key Responsibilities\n

• Conduct in-depth clinical assessments to develop individualized treatment plans tailored to meet the unique needs of each patient.

• Facilitate evidence-based therapeutic interventions, including individual and group counseling sessions that align with the latest best practices in mental health care.

• Maintain up-to-date, accurate clinical documentation in compliance with HIPAA regulations and our organization’s standards, ensuring timely access to patient records.

• Work collaboratively with a multidisciplinary treatment team, including physicians, nurses, and chaplains, to provide comprehensive care that addresses both medical and psychosocial concerns.

• Provide education and emotional support to patients and their families, empowering them to navigate the complex emotions and decisions related to hospice care.

• Stay informed on current research and developments in mental health to continuously enhance your practice and the services offered at our Hospice Center.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
