Work Environment

You will be working onsite at our modern rehabilitation facility, which is designed to support advanced therapeutic practices in a comfortable and engaging atmosphere. As part of our collaborative team, you will interact closely with nursing staff, occupational therapists, and caregivers to provide seamless care for our residents. Key Responsibilities

• Conduct comprehensive physical therapy evaluations to determine each resident's mobility, strength, and functional capabilities.
• Develop, implement, and adjust individualized treatment plans focused on pain management, mobility improvement, and fall prevention.
• Administer various therapeutic interventions, including specialized hands-on manual therapy, therapeutic exercises tailored to individual needs, and structured balance training to enhance stability.
• Assess and recommend the use of assistive devices and mobility aids to promote independence among residents.
• Engage in active collaboration with interdisciplinary teams, including nursing staff and occupational therapists, to ensure cohesive and integrated care delivery.
• Educate residents and their families on effective home exercise programs, mobility strategies, and critical safety measures to encourage ongoing health and wellness.
• Maintain precise and timely documentation of patient progress and interventions to meet Skilled Nursing Facility (SNF) regulations and standards.

Patient Population / Client Focus

In this role, you will primarily serve an elderly population within the skilled nursing facility setting, where residents often face complex health challenges, including post-operative recovery, chronic pain syndromes, and mobility restrictions associated with aging. Your expertise will be instrumental in helping them regain independence and improve their functional capacity.

Qualifications

• Licensure: Active and valid Physical Therapy license in the state of Virginia.
• Education: Doctor of Physical Therapy (DPT) or a Bachelor's or Master's degree in Physical Therapy from an accredited institution.

Preferred Experience

• At least two years of experience working in a Skilled Nursing Facility or a geriatric rehabilitation environment.
• Familiarity with long-term care regulations, best practices in rehabilitative therapy, and comprehensive care for older adult patients.
