Work Environment

This role predominantly involves working in the clients' home environments, providing a unique dynamic where therapy is adapted to the living context of the patient. As a full-time Physical Therapist, you will engage with patients Monday through Friday, collaborating closely with diverse healthcare professionals to foster therapeutic outcomes. You will receive support from our administrative team to streamline your documentation and scheduling, allowing you to focus entirely on patient care.

Key Responsibilities

• Direct the physical therapy treatment plan to prevent disabilities, relieve pain, and enhance muscle function, ensuring every therapy aligns with individual client capabilities.
• Supervise and guide Physical Therapy Assistants in the implementation of treatment plans, ensuring compliance with best practices and state regulations.
• Conduct thorough evaluations of clients' functional abilities through diagnostic tests, tailoring rehabilitation strategies to maximize outcomes.
• Assess the home environment and recommend modifications or resources to improve client independence during therapy sessions.
• Provide skilled physical therapy services in-home as prescribed by physicians, utilizing innovative techniques and modalities.
• Administer therapeutic exercises and engage clients in functional activities, motivating them to achieve their goals.
• Effectively communicate therapy goals and practices to clients and their families, educating them on device usage and therapy programs.
• Maintain meticulous documentation, including initial evaluations, care plans, progress notes, and discharge summaries, ensuring compliance with clinical standards.
• Participate actively in interdisciplinary team conferences, sharing insights and collaborating on holistic care strategies for each client.
• Monitor clients' progress, reporting any significant changes to the healthcare team while adjusting the care plan as necessary.
• Complete regular assessments of therapy effectiveness and modify the care plan based on client feedback and clinical observations.
• Remain accessible to Physical Therapy Assistants during treatment sessions, providing guidance and support as needed.

Qualifications

• Valid State PT licensure and current CPR certification.
• Bachelor's Degree in Physical Therapy or higher from an accredited institution.
• A minimum of one year of hands-on experience in a physical therapy role, preferably in a home care or acute care setting.
• Strong interpersonal skills, with an ability to engage and motivate patients and their families effectively.
