What You'll Do:
- Assess, plan, treat, and evaluate patients to develop and implement personalized care plans
- Collaborate with physicians and healthcare team members
- Provide supervision of OT Assistants and students
- Promote a safe, clean environment following best practices
- Document all patient care activities accurately and timely
- Maintain confidentiality, privacy, and professional standards
- Use evidence-based interventions to ensure quality care
- Assist patients with transfers, positioning, and activities of daily living
- Participate in ongoing education and professional development
What You Need:
- Valid Illinois Occupational Therapist license
- Minimum of 1-2 years' home health or relevant therapy experience preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
