California Institute of Health & Social Services (CIHSS) is seeking an experienced Case Manager (Family Home Specialist) to oversee compliance and quality care for neurodivergent adults in family home settings. This is a compliance-focused role requiring significant fieldwork, documentation expertise, and regulatory knowledge.

Join Our Team as a Part-Time (30 hours/week) Case Mangar (Family Home Specialist).

Conduct bi-monthly face-to-face home visits to assess health, safety, and HCBS compliance.

Develop and submit individualized service plans (ISPs), monthly reports, and clinical documentation.

Coordinate care and resources for assigned clients.

Provide crisis-response support and initiate SIR reporting per Title 17 protocols.

Ensure compliance with Title 17 regulations and HCBS-Final Rule standards.

Maintain accurate data entry in the Therap platform.

Bachelor’s degree in psychology, social work, health services, or a related field.

2-4 years of experience in developmental services case management.

Applied knowledge of Title 17 guidelines and HCBS-Final Rule compliance.

Valid California driver’s license, clean driving record, and a reliable personal vehicle for frequent field visits.

Strong written and verbal communication skills.

First Aid/CPR certification (attainable within 10 days of hire).

Pre-Employment Requirements:
