Work Environment

Key Responsibilities

• Act as a liaison between patients, families, and outpatient providers, improving communication and care coordination.
• Engage with insurance companies for pre-certification processes and utilization management to navigate financial concerns for patients.
• Participate actively in discharge planning and aftercare strategies to ensure continuous support for patients upon leaving the hospice setting.
• Provide therapeutic meal support, guiding patients in developing healthy relationships with food through supervised meal sessions.
• Deliver empathetic, affirming, and individualized care that respects the unique backgrounds and intersecting identities of each client.

Patient Population / Client Focus

The ideal candidate will mainly work with older adults facing terminal illnesses and their families, focusing on emotional and psychological support. This role demands sensitivity, patience, and a profound understanding of grief, loss, and the complexities surrounding end-of-life care, ensuring that patients receive holistic support tailored to their specific needs.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• Possession of a valid LCSW license in the state of New York is essential.
• Experience working with older adults, particularly in hospice or palliative care settings, is strongly preferred.
• Demonstrated clinical and diagnostic skills, with the ability to handle complex emotional situations effectively.
• Exceptional communication and interpersonal skills to collaborate successfully with clients, families, and team members.
• The ability to work independently as well as part of an interdisciplinary team is crucial for this position.

Preferred Experience

While the core qualifications focus on educational and licensure credentials, candidates with additional certifications in areas such as grief counseling, trauma-informed care, or mental health first aid will be viewed favorably. Compensation and Benefits

Watson Healthcare Clinic offers a competitive salary range of $90,000 to $110,000 annually, commensurate with experience.
