Key Responsibilities\n

As a Licensed Professional Counselor at our organization, you will:

• Conduct thorough ongoing assessments to identify and address the diverse needs of your patients effectively.

• Communicate regularly with supervisors, direct care staff, family members, and guardians to provide updates on an individual's progress and necessary adjustments to their care plan.

• Prepare and submit a schedule to supervisors that outlines your appointments, home visits, and observations related to monitoring patient advancement.

• Coordinate and execute home visits to gather essential data, perform chart reviews, and conduct insightful staff interviews.

• Ensure the proper implementation of individualized program plans and monitor their effectiveness in conjunction with client needs.

• Participate in interdisciplinary team meetings, sharing insights and strategies that contribute to holistic patient care.

• Document monthly and quarterly progress notes, reflecting changes in a client's condition and adjustments in treatment plans.

• Organize and facilitate staff training to ensure competency in implementing program plans and handling documentation.

• Conduct follow-up interviews and home visits to provide ongoing support tailored to the client’s evolving needs.

• Maintain accurate and organized records to ensure accessibility of information for all relevant parties.

Patient Population / Client Focus\n

You will engage with a varied patient population that includes both adults and youth, providing essential counseling services tailored to each individual's unique circumstances. Qualifications\n

The ideal candidate for this role will possess:

• An active and unrestricted license as a Professional Counselor in Oregon.

Compensation and Benefits\n
