Key Responsibilities\n

• Conduct thorough assessments to determine the mental health needs of patients.

• Develop and implement treatment plans that are tailored to individual client goals.

• Communicate regularly with supervisors, direct care staff, families, and other stakeholders regarding a client’s progress and treatment adjustments.

• Schedule and coordinate patient appointments, home visits, and therapeutic observations.

• Participate in multidisciplinary team meetings to discuss case progress and treatment strategies.

• Complete detailed monthly and quarterly progress notes to track the effectiveness of interventions.

• Lead training sessions to ensure staff members are adept in the program implementation and documentation processes.

• Maintain organized records, ensuring accessibility for all involved parties.

• Act as a liaison with local mental health organizations to assist in community outreach and development goals.

• Utilize EMR software to document client interactions and maintain confidentiality.

Qualifications\n

• Active and unrestricted LPC license in the state of Arizona.
