Key Responsibilities\n

• Develop and provide a tentative schedule for appointments and home visits, actively monitoring work advancement and addressing challenges.

• Coordinate and conduct home visits to fulfill requirements for in-home observations, chart reviews, and staff interviews, aiding in holistic care.

• Oversee the effective implementation of program plans, ensuring all staff members understand and follow established protocols.

• Engage in regular interdisciplinary team meetings to discuss client progress and any required adjustments to treatment plans.

• Maintain accurate and timely documentation, including monthly and quarterly progress notes detailing client advancements and ongoing challenges.

• Organize training sessions for colleagues to ensure staff competency in delivering program plans and maintaining documentation standards.

• Conduct follow-up assessments through interviews, home visits, and observations to provide continued support and evaluate evolving needs.

• Maintain meticulous records to ensure all relevant information is readily accessible to authorized personnel and stakeholders.

Patient Population / Client Focus\n

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Current and unrestricted licensure as a Professional Counselor (LPC) in Arizona.
