Key Responsibilities\n

• Conduct thorough evaluations to understand each patient’s unique needs and develop personalized treatment plans.

• Initiate and facilitate clear communication among supervisors, direct care staff, family members, and other relevant stakeholders to ensure holistic care.

• Document and share updates on patient appointments, home visits, and observation sessions to track each patient’s progress accurately.
\n
• Plan and conduct home visits to perform assessments, complete chart reviews, and engage in interviews for data collection and analysis.
\n
• Participate actively in interdisciplinary team meetings to collaborate on best practices and share insights about client progress.
\n
• Complete monthly and quarterly progress notes that detail therapy sessions and assess the effectiveness of treatment plans.
\n
• Organize and conduct training sessions to equip team members with the necessary skills and knowledge to implement the treatment plan effectively.

• Perform follow-up interviews and home visits to continually assess and address changing needs of clients.
\n
• Maintain up-to-date records in compliance with legal and ethical standards, ensuring information is readily accessible.

• Serve as a bridge between clients and community resources, advocating for their mental health needs.

This role will serve a diverse clientele, including adults and youth, providing therapy for various issues such as anxiety, depression, trauma, and substance abuse. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Possession of an active and unrestricted Professional Counselor (LPC) license in the state of Colorado.
