Key Responsibilities\n

• Conduct ongoing assessments of patients to address specific mental health needs with compassion and professionalism.

• Facilitate communication among supervisors, direct care staff, family members, and other stakeholders to monitor individual progress effectively.

• Develop and provide a tentative schedule for appointments, home visits, and reviews to track care advancements consistently.

• Coordinate and execute home visits for observational assessments and ensure client needs are met through in-home support.

• Guarantee that the implementation of individual program plans is efficient, adapting as necessary to maximize positive outcomes.

• Actively participate in Interdisciplinary Team Meetings and relevant discussions, providing insights into treatment strategies and progress.

• Compile and maintain accurate monthly and quarterly progress notes that reflect client developments and treatment responses.

• Organize and implement training for staff to ensure proficient understanding of program plans and necessary documentation protocols.

• Conduct follow-up home visits and interviews to provide ongoing support tailored to the client's evolving needs.

• Serve as a liaison to both the public and the mental health community, fostering development goals and strategies effectively.

Qualifications\n
