Patient Outreach Specialist

To support integrated primary care, the full-time Patient Outreach Specialist will manage and coordinate care for patient panels, monitor attribution, and facilitate outreach to ensure optimal health and wellbeing, working remotely from various locations. Key responsibilities Coordinate preventative screenings and wellness strategies for patient panels Monitor patient attribution and manage chronic disease registries to promote quality initiatives Collect and communicate patient data to identify barriers to care and support the healthcare team Required qualifications Post-secondary education of one to two years in a healthcare or medical-related field preferred Minimum of one year experience in ambulatory care, community outreach, or patient support preferred Training in Motivational Interviewing and/or Mental Health First Aid preferred Background as a Certified Medical Assistant (CMA) is preferred
