While primary hours are Monday to Friday from 8:00 AM to 4:30 PM, occasional evening or weekend hours may be necessary based on client needs, requiring a commitment to providing continuous care.

Key Responsibilities\n

• Conduct comprehensive assessments of each senior client's psychosocial needs to identify emotional, mental, and social factors impacting their well-being.

• Collaborate with clients and their families to create personalized care plans that address specific goals, preferences, and available resources.

• Provide individual and family counseling, offering emotional support and guidance through life transitions, such as loss, relocation, or health crises.

• Serve as an advocate for clients, ensuring they have access to essential community resources and services that promote independence and improve quality of life.

• Engage with interdisciplinary teams, working closely with healthcare professionals, nurses, and caregivers to coordinate care and optimize client outcomes.

• Make regular home visits to monitor client well-being, assess living conditions, and intervene in case of safety concerns.

We offer a competitive annual salary range of USD 70,000 to 100,000, depending on experience and overall qualifications.
