Key Responsibilities\n

• Collaborate with primary care providers to identify, assess, and address the mental health and psychosocial needs of patients, enhancing the overall efficacy of the care team.

• Deliver timely mental health interventions for patients in crisis, ensuring their immediate emotional support and de-escalation needs are met.

• Serve as a trusted advisor to primary care practitioners by evaluating client suitability for primary care versus specialized mental health services, providing necessary referrals when required.

• Regularly conduct follow-up assessments to monitor patient progress, providing feedback to adjust treatment plans effectively.

• Empower patients through the development and implementation of personalized relapse prevention strategies.
\n
• Identify at-risk patients proactively and collaborate to create individualized preventive health programs tailored to their unique needs.

• Oversee the delivery and coordination of mental health care services, engaging with related treatment providers to ensure a holistic approach.

• Maintain meticulous documentation of patient interactions, treatment plans, and interventions in accordance with Agency and Federally Qualified Health Center (FQHC) compliance standards.

• Ensure that open lines of communication with primary care providers are maintained, facilitating collaborative treatment planning that addresses all aspects of patient health.

• Guide patients in making healthier lifestyle choices and adhering to prescribed medication plans, addressing any adverse pharmaceutical effects they may encounter.

• Adhere to the Clinical Professionals' Code of Ethics and perform additional duties as assigned.

Patient Population / Client Focus\n

You will work with a diverse client population facing various mental health challenges, including anxiety, depression, substance use disorders, and severe emotional distress. Your work will positively impact their ability to navigate everyday challenges and improve their quality of life through effective therapeutic intervention.

Qualifications\n

• A Master’s Degree in Clinical Counseling from an accredited institution is mandatory.

• Possession of a valid LPC (Licensed Professional Counselor) certification is required.

• Strong knowledge of mental health evaluation methodologies, treatment planning, and developmental psychopathology is essential.

• Familiarity with family therapy, group therapy, child psychotherapy techniques, and DSM-IV diagnosis is preferred.

• Fluency in both English and Spanish is a plus but not required.
\n
• Must demonstrate sufficient mental and physical health capacity to manage the demands of the role.

• An understanding of the socio-economic impacts of mental health on patients and their families is necessary.

• Awareness of available community resources and services will help in providing comprehensive patient care.

Preferred Experience\n

Compensation and Benefits\n
