Travel Physical Therapist

Travel Physical Therapist needed for a 13-week home-health assignment south of Sacramento, California. Earn up to $3,380 per week and provide individualized care to improve patient mobility and independence. Work independently in patients' homes while collaborating with an interdisciplinary team.

Key Responsibilities:

• Evaluate patients in their homes and develop individualized treatment plans
• Provide skilled physical therapy services to improve mobility, strength, balance, and functional independence
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
• Monitor patient progress and modify treatment plans as appropriate
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary team members
• Complete accurate and timely electronic documentation
• Maintain compliance with home health regulations and agency policies

Qualifications:

• Doctor of Physical Therapy (DPT) or Bachelor's/Master's Degree in Physical Therapy from an accredited program
• Current California Physical Therapist License (or eligibility to obtain)
• Current BLS Certification
• Valid driver's license and reliable transportation
• Strong communication, organization, and time management skills
• Ability to work independently and manage a flexible daily schedule
