Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify clients' strengths, needs, and challenges, informing individualized treatment plans and interventions.

• Collaborate with physicians and other healthcare providers to share updates on patient progress, challenges, and emerging needs, actively participating in interdisciplinary team meetings.

• Document clinical interactions and maintain accurate and timely records, including progress notes and treatment plans, adhering to department policies and ethical guidelines.

• Provide targeted education to clients regarding mental health conditions, coping mechanisms, and self-care methods that empower them to manage their health effectively.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or a closely related field.
