Key Responsibilities\n

• Conduct thorough mental health assessments for hospice patients, taking into account their medical history, psychological state, and unique emotional challenges they may face while providing end-of-life care.

• Work collaboratively with healthcare teams to develop and implement tailored treatment plans aimed at enhancing emotional well-being and coping mechanisms to handle grief and loss effectively.

• Utilize evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT), mindfulness, and trauma-informed care, to address a variety of mental health concerns related to end-of-life issues.

• Facilitate both individual and family therapy sessions, assisting patients and their families in processing complex emotions associated with hospice care.

• Engage in crisis intervention as necessary, employing de-escalation techniques and making referrals for additional care when immediate treatment needs arise.

• Offer psychoeducation to patients and family members, outlining mental health conditions and the corresponding treatment options available, while connecting them with community resources.

• Maintain accurate and timely documentation of all patient interactions, treatment plans, and progress notes to meet legal and ethical standards.

• Participate in ongoing training and professional development activities to stay updated with the latest best practices in mental health and hospice care.

Qualifications\n

Compensation and Benefits\n

The position offers a competitive salary range of $100,000 to $120,000 per year, commensurate with experience and qualifications.
