Job Summary The Patient Navigator acts as a link between patients and care providers to facilitate clinical and supportive care services. The role is that of a single point of contact among referring providers, patients, and other clinical caregivers for the purpose of empowering patients in dealing with the disease and the treatment. ESSENTIAL FUNCTIONS Facilitate Patient and Family Engagement in Care Plans Coordinate Comprehensive Care and Support Services Quality Improvement Outreach and Relationship Management Other Duties as Assigned QUALIFICATIONS High School Diploma or GED (required) Associate Degree (preferred) Minimum 2 years of clinical or non‑clinical healthcare related experience For more than 50 years, the staff at Mary Bird Perkins Cancer Center has provided the highest quality care to our patients.
