Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify the emotional, social, and financial needs of detox patients.

• Coordinate and provide individual and group counseling sessions tailored to address issues such as adjustment to sobriety, grief, and coping strategies.

• Collaborate with interdisciplinary teams to develop and implement personalized care plans that ensure continuity of care both during and after detoxification.

• Facilitate connections with community resources and organizations that support patients post-discharge, enhancing their recovery journey.

• Provide crisis intervention and emotional support, equipping patients and their families with the tools and resources necessary for effective recovery.

• Maintain accurate and timely documentation in compliance with hospital policies and regulatory requirements, ensuring seamless communication within the care team.

• Participate actively in hospital committees focused on patient care improvements and social work initiatives, delivering expertise on psychosocial issues.

Patient Population / Client Focus\n

You will work primarily with patients undergoing detoxification from substance use, including a variety of substances. This includes conducting psychosocial evaluations to understand each patient's unique background, support systems, and obstacles that can affect their treatment and recovery. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.
