Work Environment

This is a full-time onsite position based in a home care setting, allowing you to engage directly with patients in their communities. Working hours are primarily from 9:00 a.m. to 6:00 p.m., with occasional evening clinics up to 8:00 p.m. Engagement in community outreach events and health fairs may take place on weekends, allowing you to actively participate in the fabric of your community's health.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, utilizing evidence-based practices.
• Diagnose and manage both acute and chronic medical conditions, ensuring preventive care and health promotion are at the forefront of your practice.
• Collaborate with local community organizations and stakeholders to design and execute relevant health education programs aimed at enhancing community wellness.
• Provide culturally sensitive care and act as an advocate for patients, ensuring their rights and access to high-quality healthcare services are prioritized.
• Coordinate referrals and follow-up care, ensuring a seamless continuity of care for patients under your supervision.
• Ensure meticulous maintenance of detailed medical records in compliance with relevant regulatory standards, contributing to quality assurance processes.
• Participate actively in community outreach initiatives, conducting health screenings, and educational sessions to promote awareness of health resources.

Qualifications

Compensation and Benefits
