Physical Therapist (Home Health)

Job description Job Summary P.T. Services is seeking a licensed Physical Therapist to evaluate patients, develop individualized treatment plans, and provide high-quality physical therapy services in a clinical setting. The Physical Therapist works collaboratively with patients, families, physicians, and healthcare team members to improve function, mobility, and quality of life. Key Responsibilities Perform patient evaluations and develop treatment plans.
Provide skilled physical therapy services in accordance with physician orders and professional standards.
Educate patients and caregivers on treatment plans and home exercise programs.
Monitor patient progress and modify treatment plans as needed.
Maintain accurate and timely documentation.
Ensure patient safety and compliance with all clinical policies and procedures.
Collaborate with physicians, therapists, and interdisciplinary healthcare teams.
Maintain equipment and treatment areas.
Participate in continuing education and professional development activities.
