Key Responsibilities\n

• Conduct thorough ongoing assessments to tailor interventions that meet the needs of patients, ensuring personalized and effective support.

• Facilitate communication among supervisors, direct care staff, family members, and other relevant parties to discuss progress and coordinate care effectively.
\n
• Draft tentative schedules for appointments, home visits, and clinical observations, streamlining workflow and ensuring all treatment goals are tracked.
\n
• Coordinate in-home visits to assess client environments, perform chart reviews, conduct staff interviews, and analyze data for comprehensive service delivery.
\n
• Ensure that individualized program plans are not only created but also effectively implemented and monitored for desired outcomes.
\n
• Engage actively in Interdisciplinary Team Meetings, contributing your insights and expertise to enhance collaborative treatment efforts.
\n
• Document progress notes on a monthly and quarterly basis, maintaining records that facilitate the continuity of care.
\n
• Organize and lead training sessions for team members, ensuring staff are competent in program plan execution and documentation.
\n
• Provide ongoing support for clients through follow-up home visits and interviews, adjusting plans as necessary to continue meeting individual needs.

• Act as a liaison with the broader public and mental health organizations, enhancing outreach and community connections that benefit development goals.

• Help develop procedures for training materials to enhance service delivery and program effectiveness.

Your role will encompass working with a diverse client base, including adults and youth facing a variety of mental health challenges. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Possession of an active and unrestricted licensure as a Professional Counselor (LPC) in the state of Georgia.

• Minimum of two years of experience in counseling, ideally within clinical settings where you have dealt with a range of mental health issues.

• Strong understanding of legal and ethical standards related to mental health practice.

Compensation and Benefits\n

The total annual salary for this position ranges from USD 90,000 to 100,000, commensurate with experience and caseload.
