Key Responsibilities\n

• Independently manage a designated caseload of patients as determined by the Social Work Service Department, focusing on the comprehensive evaluation of social, emotional, financial, and cultural influences on their care.

• Conduct thorough assessments to identify risks and develop tailored treatment and intervention plans aimed at addressing patients' psychosocial needs.

• Lead multidisciplinary team meetings to facilitate discussion on patient progress, treatment plans, and crisis intervention strategies, ensuring a coordinated approach to addressing complex psychosocial issues.

• Educate healthcare staff and community partners regarding the role of social work in healthcare, emphasizing the impact of psychosocial issues on patient care and outcomes.

• Supervise and mentor junior staff, including Social Work Interns and volunteers, providing weekly guidance and feedback to cultivate their professional development.

• Work collaboratively with case managers to ensure seamless coordination of care, providing resources, education, and referrals to patients and their families to address any identified needs.

• Engage in community outreach by connecting patients with resources and support services to facilitate their ongoing care and prevent hospital readmissions.

• Participate in on-call rotations to respond promptly to crises and support the healthcare team during critical incidents.

• Regularly attend team rounds and patient care conferences to stay updated on case assignments and contribute to care strategy discussions.
