Independent Living Specialist - Vanderbilt Stabilization

Duties/Responsibilities Provide comprehensive case management services to identified chronic homeless individuals. Responsible for all aspects of the assigned caseload, including meeting with clients on a regular basis both in the field and in the office, and ensuring that all clinical files and reports are complete and up to date. Conduct comprehensive assessments to identify barriers to housing, assist clients to overcome housing barriers and secure entitlements, develop and work with clients to progress on individualized housing plans, build a network of support services for clients. Prepare clients for housing interviews and arrange housing appointments, and participate in case conferences as necessary. Liaise with clinical staff in other BRC or outside programs regarding common clients. Provide emergency first aid/CPR or overdose prevention protocol assistance when needed. Perform related duties as assigned. Hours Full time – 37.5 hours per week Tuesday – Saturday 12:00 pm – 8:30 pm Qualifications High School Diploma/GED required; Bachelor's degree preferred and Master's ideal. Bilingual Spanish strongly preferred but not required. Excellent communication, writing skills, and computer proficiency required. Experience working in residential treatment and with the MICA and homeless populations. Knowledge of HRA and NY/NY application systems a plus. CPR training certification or willingness to take training class in CPR.
