Description

• Assess patients in both inpatient and outpatient environments and craft tailored treatment plans that meet their individual needs
• Deliver physical therapy interventions focused on enhancing mobility, strength, balance, and overall function
• Collaborate effectively with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimal patient outcomes
• Inform and educate patients and their families about therapeutic objectives, discharge processes, and home exercise routines
• Maintain thorough documentation of evaluations, treatments, and advancements in line with facility regulations
• Engage in multidisciplinary care team meetings and conferences to provide well-coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
