Overview Good Shepherd Hospice is seeking a compassionate Hospice Intake Coordinator to serve in our Houston and surrounding communities.The Hospice Intake Coordinator assists in gathering required patient information and coordinating enrollment of new patients into hospice program.Hospice Admissions/Intake experience requiredHomecare Homebase software experience highly preferred Responsibilities Initial contact person for all new inquiries and referralsResponds promptly to any referral or inquiryCoordinates the scheduling of intake personnel to follow up with referralsProvides guidance to hospice staff during the referral and enrollment processInitiates communication with the RN Case Manager regarding eligibility and enrollment into hospice programInforms the RN Case Manager of unusual or potentially problematic patient/family issuesCoordinates all admission enrollment paper workObtains history and physicals, progress notes, lab, nurse’s notes, medication sheets, etc. as needed to complete initial patient assessmentMaintains regular communication with the referral sourceReviews completed enrollment packet for accuracy and forwards for billing and filingInputs initial referral information into the computerCommunicates with medical professionals, the hospice team and othersCoordinates completion of admission packetsMaintains accurate, up-to-date recordsDemonstrates strong communication and customer service skills Qualifications Hospice Intake experience requiredValid driver’s licenseDemonstrated commitment to the hospice philosophy of careCan successfully pass background checks & drug screenJob SummaryCategory: IntakePosition Type: Full-TimeSchedule: Hidden (37183)Shift Hours: 8a-5pRemote: No
