Work Environment

This Nurse Practitioner position is an onsite role where you will visit patients in their homes across Houston. You will work closely with a multidisciplinary team, which may include registered nurses, physical therapists, and social workers. Using advanced nursing knowledge and clinical judgment, you will engage with patients and their families in a holistic approach to health care. This position will average around 36 to 40 hours per week, with flexible scheduling to accommodate patient needs and ensure a healthy work-life balance.

Key Responsibilities

• Conduct thorough assessments and obtain detailed medical histories from patients during home visits, identifying both acute and chronic health concerns.
• Formulate, implement, and evaluate evidence-based treatment plans in collaboration with the broader healthcare team to ensure optimal patient outcomes.
• Order, conduct, and interpret diagnostic tests, and prescribe medications and therapies based on established protocols and patient needs.
• Educate patients and their families on managing health conditions, medication compliance, and understanding care plans, while providing guidance on lifestyle modifications.
• Participate in regular interdisciplinary team meetings to review patient progress, share insights, and coordinate care to improve health outcomes.
• Utilize Electronic Health Record (EHR) systems to document patient encounters, track care plans, and maintain accurate medical records, ensuring compliance with regulations and standards.
• Contribute to continuous quality improvement initiatives and integrate evidence-based practices into home care protocols.
• Stay current with developments in nursing and healthcare regulations to maintain high standards of care and patient safety.

Patient Population / Client Focus

The primary focus of this role encompasses a diverse patient population, including elderly individuals, those with debilitating chronic illnesses, and patients recovering from surgery or acute conditions. By providing comprehensive and individualized care in their homes, you will empower clients to manage their health effectively and enhance their quality of life.

Qualifications

• Current certification as a Nurse Practitioner, with an active state nursing license in Texas.
• A Bachelor’s degree in Nursing is required; a Master’s degree in Nursing from an accredited program is preferred.
• At least two years of clinical experience as a Nurse Practitioner, with direct experience in a home care or hospital setting being advantageous.
• Exemplary communication skills, capable of building rapport with patients, families, and healthcare team members.
• Strong analytical capabilities and the ability to think critically in fast-paced and evolving environments.

Preferred Experience

• Experience with EHR systems and familiarity with home health care protocols.
• Specialized experience in geriatric care or chronic disease management is highly valued.
• Proficiency in patient education, addressing diverse health literacy levels among clients.

Compensation and Benefits
