Work Environment

This position is based in Fort Myers, Florida, where you will be working in the home care setting. Amedisys Physical Therapy emphasizes a drug-free workplace and requires various vaccinations as a part of our employment standards. You will work a 40-hour week with variability in weekends, ensuring that our patients receive care when they need it most.

Key Responsibilities

As a Physical Therapist, your key responsibilities will include:

• Conducting comprehensive evaluations to determine the physical capabilities and needs of each patient.
• Developing and implementing individualized treatment plans that promote the patients' functional independence and quality of life.
• Coordinating with the interdisciplinary treatment team, including physicians and referral sources, to ensure a seamless continuum of care.
• Educating patients and their families about treatment plans, goals, and follow-up care, fostering a collaborative approach to health management.
• Maintaining accurate documentation of treatment provided, progress notes, and communication with the healthcare team.
• Participating in community outreach efforts and quality improvement initiatives, enhancing the visibility and reputation of Amedisys Physical Therapy.
• Supervising and mentoring support staff, including Physical Therapy Assistants and students, and contributing to their professional development.

Patient Population / Client Focus

You will be working primarily with a diverse patient population in their homes, including clients recovering from surgery, geriatric patients with chronic illnesses, and individuals with disabilities. The ability to interact respectfully and effectively with patients from various cultural backgrounds is essential, as your approach will impact their recovery and engagement.

Qualifications

To be considered for this role, candidates must meet the following qualifications:

• A Bachelor's Degree in Physical Therapy.
• A valid state license in Physical Therapy.
• Strong written and verbal communication skills to facilitate effective interaction with patients and the healthcare team.
• Proficiency in computer systems and documentation processes relevant to patient care.

Preferred Experience

We prefer candidates who have:

• A minimum of 2 years of experience as a Physical Therapist, ideally with home care or outpatient experience.
• A background in community outreach or program development within a healthcare setting.
• Experience in supervising and working collaboratively with support staff, fostering a team-oriented environment.

Compensation and Benefits
