Work Environment

This full-time position is onsite, focusing on delivering therapy services directly in patients' homes throughout North Fort Myers, Florida. Key Responsibilities

• Conduct thorough evaluations of patients, including the review of medical histories and the administration of targeted assessments to develop tailored treatment plans.
• Engage patients and their families by clearly communicating evaluation results and collaboratively setting achievable goals that enhance the patients' rehabilitation journey.
• Implement treatment protocols effectively, utilizing your clinical judgment to adjust approaches based on patient progress and the dynamic healthcare landscape.
• Maintain open communication with interdisciplinary healthcare teams, recognizing changes in patient conditions that may necessitate adjustments in treatment plans or referrals for additional medical consultations.
• Document patient care processes meticulously, including initial evaluations, daily treatment notes, progress summaries, and discharge evaluations, ensuring compliance with institutional standards and regulations.
• Manage time efficiently, adhering to productivity expectations while ensuring quality care. Participate in meetings and community outreach to foster accessibility to services.
• Contribute to departmental goals by supporting quality improvement initiatives and adhering to regulatory standards.
• Pursue ongoing education and competencies to enhance professional growth and mentor aspiring clinicians through supervisory roles.
• Deliver outstanding patient experiences through a high level of flexibility and responsiveness to individual needs and circumstances.
• Contribute to the improvement of access to care by working with the team to identify and mitigate barriers for patients.

Patient Population / Client Focus

In this role, you will be providing care to a diverse population of patients including those recovering from surgery, managing chronic illnesses, or seeking rehabilitation after injuries. Your experience and empathy will be crucial in helping patients navigate their recovery journeys within the comfort of their homes.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid Physical Therapy license in the state of Florida.
• Current Basic Life Support (BLS) certification from the American Heart Association.
• Proficient understanding of healthcare regulations as they pertain to home care services.

Preferred Experience

• At least 2 years of experience in a physical therapy setting, ideally with a focus on home care.
• Prior experience collaborating with interdisciplinary teams would be advantageous.
